Made on your phone, sent by you
Tap Report for your doctor (PDF) in Settings › Your data. On Android you save it to a file you choose; on iPhone the share sheet opens. Nobody gets it unless you give it to them.
What it isn’t
The report lists what you logged. It doesn’t say whether something is wrong — that’s for your doctor to look at with you.
